摘要:
儿童感染后闭塞性细支气管炎(PIBO)是一种罕见的慢性阻塞性肺部疾病,近年来其发病率呈上升趋势,但目前国内外尚无统一且特效的治疗方案,主要是经验性治疗,即对症支持与抗炎治疗相结合。综合现阶段对PIBO治疗方法的认识,可分为药物治疗和非药物治疗。药物治疗包括肾上腺糖皮质激素、大环内酯类抗生素、支气管扩张剂、白三烯受体拮抗剂、间充质干细胞(MSCs)和单克隆抗体类等。其中,因糖皮质激素具有较强的非特异性抗炎和免疫抑制作用,使其成为治疗PIBO的基础药物及首选药物。MSCs和单克隆抗体类药物是治疗闭塞性细支气管炎综合征(BOS)的新药,其有望成为治疗PIBO患儿的潜在药物。此外,还包括营养支持、呼吸支持、肺部理疗、肺康复训练和肺切除等非药物治疗方法。现就近年来国内外各中心对PIBO治疗方法的研究进展进行阐述,为临床治疗PIBO患儿提供参考。
