儿童过敏性紫癜血浆一氧化氮与内皮素-1变化的意义

Significances of changes of nitric oxide and endothelin-1in plasma of children with Henoch-Schonlein′s purprua

摘要: 目的:探讨内源性一氧化氮(NO)和内皮素-1(ET-1)在儿童过敏性紫癜(HSP)发病中的作用。 方法:对48例HSP患儿[紫癜性肾炎(HSPN)组21例,非肾脏受累(NHSPN)组27例] 急性期、恢复期血浆NO、ET-1水平进行测定。 结果:HSP患儿急性期血浆NO、ET-1水平均明显高于恢复期及对照组(P<0.01),尤以HSPN组升高更显著,与NHSPN组比较差异有显著性(P<0.05);急性期血压升高组NO/ET-1比值较血压正常组低(P<0.05);恢复期非肾脏受累组NO、ET-1水平虽仍较对照组高,但差异无显著性(P>0.05);HSPN组恢复期NO、ET-1水平虽较急性期下降,但仍明显高于对照组(P<0.05),且NO/ET-1比值小于急性期;HSPN组和NHSPN组急性期和恢复期NO与ET-1呈明显正相关(HSPN组急性期r=0.7843,P<0.01,恢复期r=0.4920,P<0.01;NHSPN组急性期r=0.6831,P<0.01,恢复期r=0.6317,P<0.01)。 结论:NO、ET-1的水平可作为临床判定HSP患儿病情轻重、并发症的发生及预后的重要指标。

Abstract: Objective To investigate the effects of nitric oxide(NO) and endothelin-1(ET-1) on the children with Henoch-Schonlein′s purpura(HSP). Methods The plasma NO and ET-1 levels of 48 children with HSP,inclnding 21 children with HSP nephritis (HSPN) and 27 children with non-HSPN (NHSPN), were determined both in acute and recovery phases. Results The concentrations of the plasma NO and ET-1 in acute phase were much higher than those in recovery phase and control group, respectively (P<0.01), and the changes were much more significant in children with HSPN than those without HSPN. The ratio of NO/ET-1 of hypertension group was lower than that of children without hypertension in acute phase (P<0.05). There were no significant differences of the plasma NO and ET-1 between NHSPN group in recovery phase and the control group(P>0.05). However, the plasma NO and ET-1 concentrations of the HSPN group in recovery phase were still higher than those in the control group (P<0.05),and were lower than acute phase, and the ratio of NO/ET-1 in recovery phase was lower than that in acute phase. There was a positive correlation between NO and ET-1 in children with HSP (HSPN group in acute phase:r=0.7843,P<0.01,in recovery phase:r=0.4920,P<0.01);NHSPN group in acute phase r=0.6831,P<0.01,in recovery phase r=0.0.6317,P<0.01). Conclusion The levels of NO and ET-1 are important indexes to determine the state of illness, occurrence of complication and prognosis in children with HSP.
