心肌应变率成像对高血压患者左室局部舒张功能的定量评价

Evaluation of regional diastolic function in patientswith hypertension using strain rate imaging

摘要: 目的:探讨心肌应变率成像(SRI)技术定量评价高血压患者左室局部舒张功能的意义。 方法:应用SRI技术对22名健康志愿者和30例高血压病患者进行检测,获取左室各节段心肌长轴方向的应变率曲线,分别测量收缩期、等容舒张期、舒张早期和心房收缩期的最大应变率(SR),对两组结果进行对比分析。 结果:与正常组比较,高血压患者收缩期除侧壁基底段外其余各节段心肌SR值降低(P<0.05), 等容舒张期除后间隔基底段和中间段及侧壁基底段外其余各节段心肌SR值均明显降低(P<0.05),舒张早期除侧壁基底段外其余节段心肌SR值均明显降低(P<0.05),心房收缩期各节段心肌SR测值无明显改变。 结论:SRI可敏感地检出高血压病患者左室局部舒张功能的异常,是准确、定量评价局部心肌舒张功能的一种新检测方法。

Abstract: Objective To explore the clinical values of strain rate imaging (SRI) in detection of regional function of left ventricular in patients with hypertension. Methods Twenty-two healthy volunteers and 30 patients with hypertension were examined with SRI. Strain rates of left ventricular segments along axis were analyzed by strain curves, and the peak SR at systole, isovolumic relaxation phase, early diastole and atrial contraction were calculated. Results Compared with the healthy subjects, patients with hypertension had appreciably lower SR on all walls in systole (P<0.05), except the basal of the lateral ones. SR decreased on all walls in isovolumic relaxation phase (P<0.05), except the basal and middle of the post septum and the basal of the lateral ones. SR decreased on all walls in early diastole (P<0.05),except the basal of the lateral ones, while the peak SR at atrial contraction showed no significant differences in patients with hypertension. Conclusion SRI allows sensitive distinguishing between abnormal and normal regional diastolic function of left ventricular, therefore provides a new method for accurate evaluation of regional diastolic function of left ventricular. strain rate,hypertension/ultrasonography, ventricular function;left, ultrasonography;doppler;color
