韩国与中国延边地区宫颈病变组织中人乳头瘤病毒感染类型比较

Comparison of human papillomavirus genotypes in uterine cervical cancer between South Korea and Yanbian area of China

摘要:

目的:对比研究中国北方延边地区与韩国宫颈病变组织中人乳头状瘤病毒(HPV)类型的分布特点。方法:选取韩国高丽大学医疗中心病理科存档宫颈癌前病变和鳞癌、腺癌的蜡块标本339例,同时选取延边大学医院以及延边妇婴医院病理科存档宫颈上皮内瘤变(CIN)和鳞癌、腺癌的蜡块标本296例。常规HE染色观察标本,并选取适当蜡块进行DNA提取,利用HPV寡核苷酸cDNA基因芯片技术对提取到的DNA进行基因分型。结果:中国延边地区和韩国宫颈病变组织中高危型HPV感染率差异无显著性,其中CIN-1患者中HPV感染率分别为32.6%和34.6%;CIN-2患者中HPV感染率分别为52.6%和54.1%;CIN-3患者中HPV感染率分别为59.7%和63.2%;宫颈鳞癌患者中HPV感染率分别为94.0%和95.3%;宫颈腺上皮内瘤变(CGIN)患者中HPV感染率均为66.7%;宫颈腺癌患者中HPV感染率分别为85.2%和87.1%。HPV感染类
型也有共同特点,鳞癌组织中HPV感染的主要类型是16型,其次是18型;而在宫颈腺癌中HPV16和18型均为主要感染类型,但HPV18阳性率略高于HPV16型。韩国的宫颈癌标本中HPV感染的类型较多,可见HPV16,18,31,33,52,58和66等多种类型的感染,而且可以检测到较多的HPV混合感染的病例(CIN-2 9.1%,CIN-3 16.7%,鳞癌14.9%,腺癌14.8%)。结论:在韩国和中国北方延边地区,HPV16型是宫颈鳞状细胞癌发生中最常见的类型,而HPV18型和HPV16型均是宫颈腺癌的主要感染类型,其中HPV18型的感染率在宫颈腺癌中略高于HPV16型。与中国延边地区相比,韩国的宫颈癌组织中HPV感染的类型较多,而且存在较多的HPV混合感染的病例。

Abstract:

Abstract:Objective To investigate the distribution of human papillomavirus (HPV) genotypes between Yanbian area of China and South Korea.Methods 339 cases of uterine cervical cancer and its precursors were selected from Dept.of Pathology,Korea University Medical Center.Meanwhile,296 cases of uterine cervical cancer and its precursors,cervical intraepithelial neoplasia (CIN) were selected from Dept.of Pathology,Yanbian University Hospital and Yanbian Women’s Hospital.All of these cases were stained by HE,and appropriated blocks were selected for DNA extraction.And HPV genotypes were performed by oligonucleotide microarray (DNA chip).Results In Yanbian area of China,the positive rates of high-risk HPV were 32.6% in CIN-1,52.6% in CIN-2,54.7% in CIN-3,94.0% in squamous cell carcinoma(SCC),66.7% in cervical glandular intraepithelial neoplasm (CGIN)and 85.2% in adenocarcinoma. In South Korea,the positive rates of high-risk HPV were 34.6% in CIN-1,54.1% in CIN-2,63.2% in CIN-3,95.3% in SCC,66.7% in CGIN and 87.1% in adenocarcinoma.There was no significant difference between Yanbian of China and South Korea.HPV16 was the most common type in SCC,and HPV18 was followed.However,both HPV18 and 16 were the main types in cervical adenocarcinoma,and the positive rate of HPV18 was slightly higher than that of  HPV16.The multi-types of HPV infection cases were found in South Korea compared with  Yanbian area of China,such as the mixed infection of HPV16,18,31,33,52,58,66 and so on.The positive rates of multi-types HPV infection were 9.1% in CIN-2,16.7% in CIN-3,14.9% in SCC and 14.8% in adenocarcinoma in South Korea.Conclusion  HPV16 is the most common type in SCC of the cervix in both Yanbian area of China and South Korea,however both HPV18 and 16 are main types in adenocarcinoma of the cervix,in which the infection rate of HPV18 is slightly higher that of  than HPV16. Meanwhile,the cases of multi-types HPV infection are more frequently observed in South Korea than in Yanbian area of China.
