强化钙和维生素D饮品对儿童骨组织发育影响的前瞻性研究

摘要:

目的:探讨补充强化钙和维生素D饮品对儿童骨组织生长发育的影响,阐明发育期儿童补充营养素的必要性。方法:选取年龄为6~8岁的小学一、二年级儿童294人作为研究对象,将所有研究对象分为2组,干预组174人,对照组120人,其中男生147人,女生147人;干预组和对照组均来自同一学校的不同班级,有相似的社会经济背景。干预组每天上午饮用250 mL饮品;对照组保持日常饮食,不补充饮品,干预期1年。干预结束后观察干预前后2组研究对象钙代谢、骨矿含量、骨密度和股面积等指标的变化。结果:干预前2组钙代谢、骨矿含量 、骨密度和股面积等指标比较差异均无统计学意义(P>0.05);干预后,干预组男生和女生的25羟基维生素D[25-(OH)D]水平、骨钙素水平高于对照组(P<0.05);干预组男生下肢、股骨总量和股骨大转子骨矿含量、股骨总面积、下肢和大转子的骨密度显著高于对照组(P<0.05),干预组女生上肢骨矿含量和骨密度显著高于对照组(P<0.05)。 结论:补充强化钙和维生素D饮品可促进儿童骨组织生长发育,提示儿童生长发育期应供给充足的营养素。

Abstract:

Objective To explore the effect of calcium and vitamin D fortified drinking supplementation on the growthdevelopment of bone tissue in children,and to clarify the necessity of nutrients supplementation in childhood. Methods 294 students  (147 males,147 females) in   the grade one and two  in primary school aged between 6-8 years old  were selected and  were randomly divided in to two groups:intervention group(n=174) and control group(n=120).The students in intervention group and control group were from the same school but different classes,who had a similar social and economic background.The students in intervention group drank 250 mL drink every morning;and the students in control group maintained their daily diet without the supplement drinks.The intervention time was one year.After the intervention,the calcium metabolism,bone mineral composition (BMC),bone mineral density (BMD),bone area (BA) and so on were observed before and after  intervention in two groups.Results Before  intervention,the indexes including the calcium metabolism,BMC,BMD,BA and so on had no statistical significances between two groups(P>0.05).After  intervention,the 25-(OH)  D levels and osteocalcin levels of boys and girls in intervention group were significantly higher than those in control group (P<0.05);the total amount of lower limbs and femur,BMC of greater trochanter of femur,BA of femur and BMD of lower limb and greater trochanter of  boys in intervention group were significantly higher than those in control group (P<0.05), the BMC of bones of upper limbs and BMD of girls in intervention group were significantly higher than those in control group (P<0.05).Conclusion Supplement of drinks fortified with calcium and vitamin D could promote the growth development of children’s bone tissue.
