硝酸银和滑石粉对兔胸膜的固定作用

Effects of pleurodesis with silver nitrate and talc in rabbits

摘要: 目的:对比研究胸腔内注入硝酸银和滑石粉对兔胸膜的固定效果。 方法:分别采用0.5%硝酸银溶液和滑石粉匀浆(400 mg•kg-1)对日本大耳白兔进行实验性胸膜固定,检测术后1及3 d胸液量、胸液中白细胞计数(WBC)、多形核中性粒细胞百分比(PMN%)和乳酸脱氢酶(LDH)含量。比较术后1、3、7、14及28 d硝酸银和滑石粉匀浆组胸膜黏连情况。 结果:硝酸银组术后1及3 d胸液量及WBC高于滑石粉组(P<0.001),且两组术后3 d兔胸液量及WBC均高于1 d(P<0.001);两组术后1 d PMN%高于3 d(P<0.01),硝酸银组术后3 d PMN%显著低于滑石粉组(P<0.001)。两组术后1 d LDH均高于3 d (P<0.001),而硝酸银组术后1和3 d胸液LDH含量高于滑石粉组(P<0.01)。术后胸膜黏连评分硝酸银组高于滑石粉组(P<0.05)。 结论:0.5%硝酸银溶液和滑石粉匀浆均能诱导实验性兔胸膜固定作用,硝酸银胸膜固定效果好于滑石粉。

Abstract: Objective To study the effects of pleurodesis with the intrapleural injection of silver nitrate and talc in rabbits. Methods Intrapleural injection of 0.5% silver nitrate and talc slurry (400 mg•kg-1), respectively, were performed to Japanese white rabbits to produce pleurodesis, the pleural effusion volume, white blood cell (WBC) numbers, the percentage of polymorphonuclear neutrophil (PMN%) and the content of lactate dehydrogenase (LDH) of 1 and 3 d after intrapleural injection were examined. The pleural adhesion scores of 1,3,7,14,28 d after pleurodesis in silver nitrate group and talc group were compared. Results The pleural effusion volume and WBC in silver nitrate group were higher than those in talc group at 1 and 3 d after pleurodesis (P<0.001), effusion volume and WBC at 3 d in the two groups were higher than those at 1 d (P<0.001); PMN% in both groups at 1 d were higher than those at 3 d (P<0.01), but it was lower at 3 d in silver nitrate group compared with talc group (P<0.001); the LDH contents in both groups at 1 d were higher than those at 3 d (P<0.001), and the LDH contents at 1 and 3 d in silver nitrate group were higher than those in talc group (P<0.01). The pleural adhesion score in silver nitrate group was higher than that in talc group (P<0.05). Conclusion Both silver nitrate and talc slurry can produce pleurodesis in rabbit pleura, and the low concentration of silver nitrate is superior to talc slurry.
