斑点追踪超声心动图在心肌梗死模型大鼠检查中的应用及其意义

Application of speckle tracing echocardiography in examination of rat models of myocardial infarction and its significance

摘要: 目的:探讨斑点追踪超声心动图在心肌梗死模型大鼠检查中的意义,阐明斑点追踪超声心动图对鉴定大鼠心肌梗死的可靠性和应用价值。方法:将50只大鼠随机分为假手术组和模型组,每组25只,采用缺血预处理方法建立心肌梗死模型。对2组大鼠进行斑点追踪超声心动图检查,分析心肌缺血后大鼠超声心动图变化,采用Masson染色观察大鼠心脏组织形态表现,计算心肌梗死面积百分率。结果:与假手术组比较,模型组大鼠建模前超声心动图各指标比较差异均无统计学意义(P>0.05)。与假手术组比较,模型组大鼠左心室射血分数(EF)和环向应变(CS)、心脏前间隔CS峰值、前壁CS峰值、前侧壁CS峰值、收缩期CS峰值和前壁径向应变(RS)均明显降低(P<0.05),前壁标准化到达CS峰值时间(TTP)明显增加(P<0.05)。Masson染色,假手术组大鼠心肌组织无异常改变,未见梗死灶;模型组大鼠心肌梗死面积百分率为(4.52±1.41)%,均位于心脏前壁,为小面积的心肌细胞病变。结论:由斑点追踪超声心动图RS和TTP可确定心肌梗死及其附近区域,可定量检测大鼠心肌梗死部位。斑点追踪超声心动图可用于临床快速诊断心肌梗死及确定梗死面积。

Abstract: Objective:To investigate the significance of speckle tracing echocardiography in the examination of the rat models of myocardial infarction, and to elucidate the reliability and clinical application of speckle tracing echocardiography in the identification of the myocardial infarction rats. Methods:A total of 50 rats were randonly divided into sham operation group and model group,with 25 rats in each group. Ischemic preconditioning method was used to establish the myocardial infarction models. Speckle tracking echocardiography and cardiac tissue Masson staining were performed to analyze the changes of echocardiography and the morphology of myocardium tissue of the rats. Results:Compared with sham operation group,the each index in echocardiography of the rats in model group before modeling had no significant differences (P>0.05). Compared with sham operation group, the left ventricular ejection fraction (EF), circumferential strain (CS), the peak values of CS in the anterior septum, anterior wall, anterior lateral wall, systolic period, radial strain (RS) of the anterior wall of the rats in model group were decreased(P<0.05); the standardized time to peak of CS of anterior wall(TTP) was increased(P<0.05). The results of Masson staining showed that there was no change in the myocardium tissue and no infarction of the rats in control group;the infarction area of the rats in model group was (4.52±1.41)%;the infarction located in the anterior wall which was the myocardiocyte lesion in the small area. Conclusion:Speckle tracing echocardiography RS and TTP can reveal the infarction area and its surrounding areas of myocardial infarction, and quantitatively detect the myocardial infarction site in the rats. Speckle tracing echocardiography can be used for the clinical rapid diagnosis of myocardial infarction and confirmation of infarction area.
