万古霉素骨水泥力学性能及洗提特性

摘要: 目的:探讨万古霉素加入国产骨水泥后的力学(机械性能)改变和缓释情况,以期指导临床应用。方法:依万古霉素含量不同从0 g开始每组万古霉素含量增加0.5 g直至3.0 g共分为7组,分别与40 g骨水泥混合。应用万能力学实验机测定万古霉素骨水泥的压缩强度、弯曲模量和弯曲强度,并进行洗提实验。结果:40 g骨水泥加入0~3.0 g万古霉素洗提前的压缩强度无明显变化,洗提后含3.0g万古霉素的骨水泥压缩强度及弯曲强度均低于对照组(P<0.05),而各组万古霉素骨水泥的弯曲模量无明显变化。洗提实验中,各组在第1周释放万古霉素的量与骨水泥中万古霉素的含量呈递增关系。1周后万古霉素含量为1.5~2.0 g时,骨水泥中释放万古霉素的量明显高于其他各组(P<0.05),并更趋于稳定。结论:万古霉素能有效地从骨水泥中持续释放,在万古霉素含量的选择上,可将40 g骨水泥中加入1.5~2.0 g万古霉素作为首选剂量,并且骨水泥的力学和物理性能有增强的可能。

Abstract: Abstract:Objective To study the effects of vancomycin-impregnated polymethylmethacrylate (VCM-PMMA) on mechanics alteration and delayed release in order to guide clinical application.Methods The specimens of bone cement (each of 40 g) were divided into 7 groups depending on mixed different contents with vancomycin,containing 0,0.5,1.0,1.5,2.0,2.5 and 3.0 g vancomycin.The compressive strength,bending strength and bending elastic moduls were measured respectively.The elution examination of vancomycin from bone cement was performed by chromatography.Results There were no differences of compressive strength before elution of bone cement that were included from 0 to 3.0 g vancomycin.When the vancomycin in the content of the bone cement attained 3.0 g,the compressive strength after elution and bending elastic were lower than those in control group(P<0.05).Bending modulus of every group had no difference. In the release test,the elution rate in every group increased progressively with the content of vancomycin in polymethylmethacrylate at first week.When the vancomycin in the content of bone cement attained from 1.5 to 2.0 g after a week,the quantity of the released vancomycin was higher than the others(P<0.05),and it was gradually in stability.Conclusion Vancomycin can be released from the bone cement utility.1.5-2.0 g vancomycin are optimal doses to be impregnated in 40 g bone cement and they can improve the mechanical property of the bone cement.
