重组人促红细胞生成素对慢性肾功能衰竭患者红细胞免疫的影响

摘要: 目的: 研究重组人促红细胞生成素(r-HuEPO)对慢性肾功能衰竭患者体内红细胞免疫的影响。方法:45例慢性肾功能不全肾衰竭期(CRF)患者根据治疗情况分为初诊未治疗患者20例(对照组)及规律使用r-HuEPO 1个月以上患者25例(观察组)。应用花环结合率方法测定患者红细胞C3b花环结合率(RBC-C3bRR)及红细胞免 疫复合物花环率(RBC-ICRR ),分析血红蛋白(Hb)、血肌酐(CRE)的浓度与红细胞C3b花环结合率的相关性。 结果: 观察组Hb、RBC-C3bRR明显高于对照组 (P<0.05);观察组的RBC-ICRR明显低于 对照组(P<0.05);Hb浓度与RBC-C3bRR呈正相关(r=0.56,P<0.05);CRE浓度与RBC-C3bRR呈负相关(r=-0.55,P<0.05)。 结论:使用r-HuEPO纠正CRF患者贫血后,可以提高CRF患者红细胞膜上C3b受体活性,降低体内CRE水平,提高红细胞膜C3b受体 活性,提高CRF患者红细胞免疫功能。

Abstract: Objective To study the effects of recombinant human erythropoietin (r-HuEPO) on RBC immunity in  chronic renal failure (CRF) patients. Methods Forty-five CRF patients were divided into two groups : 20 patients without r-HuEPO (control group) and 25 patients with r-HuEPO more than one month (experimental group). The RBC-C3bRR and RBC-ICRR in two groups were counted under microscope and the correlations between HB or CRE and RBC-C3bRR were calculated. Results The concentrationd of Hb, RBC-C3bRR in experimental group were much higher than those in control group(P<0.05). The RBC-ICRR in experimental group was much lower than that in control group(P<0.05). There was a positive correlation between HB and RBC-C3bRR(r=0.56,P<0.05). There was a negative correlation between CRE and RBC-C3bRR(r=-0.55,P<0.05). Conclusion r-HuEPO can improve the anaemia in CRF patients,improve the activities of C3b receptors, improve the RBC immunity and depress the CRE concentration.
