结直肠癌组织中Ki-67和COX-2的表达及其临床意义

Expressions of COX-2 and Ki-67 in colorectal carcinoma tissue and theirclinical significances

摘要:

目的:探讨结直肠癌组织中Ki-67和环氧合酶2(COX-2)的表达及其临床意义,为临床诊治结直肠癌提供依据。方法:选取结直肠癌患者71例。采用免疫组织化学SP法检测71例结直肠癌组织(结直肠癌组)、71例结直肠癌癌旁组织(癌旁组)、20例正常大肠黏膜组织(正常组)中Ki-67和COX-2的阳性表达率,分析结直肠癌患者癌组织中Ki-67和COX-2表达与结直肠癌转移、分期及预后的关系。采用Spearman相关分析法分析Ki-67及COX-2表达的相关性。结果:结直肠癌组织中Ki-67和COX-2阳性表达率明显高于癌旁组织和正常结肠黏膜组织(χ2=24.581,P<0.05;χ2=11.538,P<0.05)。有淋巴结转移的患者癌组织中Ki-67和COX-2表达阳性率明显高于无淋巴结转移患者(χ2=5.021,P=0.025;χ2=7.289,P=0.007);Ⅰ+Ⅱ期患者癌组织中Ki-67和COX-2阳性表达率明显低于Ⅲ+Ⅳ分期患者(χ2=8.570,P=0.003;χ2=11.409,P=0.001);患者癌组织中Ki-67与COX-2表达水平呈正相关关系(r=0.520,P<0.05);随着COX-2和Ki-67表达水平的增加,患者术后中位生存时间逐渐缩短,Ki-67和COX-2阳性表达患者的1、3和5年生存率逐渐降低。结论:Ki-67和COX-2异常表达与结直肠癌的发生发展有密切关联,其表达上调参与肿瘤形成、发展、侵袭和转移,可作为判断结肠癌生物学行为的临床参考指标。

Abstract:

Objective: To investigate the expressions of Ki-67 and cyclooxygenase-2(COX-2) in colorectal carcinoma tissue and their clinical significances,and to provide basis for the diagnosis and treatment of colorectal carcinoma.Methods: A total of 71 colorectal carcinoma patients were selected.The positive expression rates of Ki-67 and COX-2 in colorectal carcinoma tissue (colorectal cancer group,71 cases),para-carcinoma tissue(para-carcinoma group,71 cases),and normal mucosal tissue (normal group,20 cases) were detected by immunohistochemical SP method.The relationship between the expressions of Ki-67 and COX-2 and metastasis,different stages and prognosis of colorectal carcinoma were analyzed.The correlation between the expressions of Ki-67 and COX-2 was analyzed by Spearman analysis method.Results: The psitive expression rates of COX-2 and Ki-67 in colorectal carcinoma tissue were obviously higher than those in adjacent tissue and normal mucosal tissue(χ2=24.581,P<0.05;χ2=11.538,P<0.05).The positive exprssion rates of COX-2 and Ki-67 in carcinoma tissue of the patients with lymph node metastasis were significantly higher than those in the patients without lymph node metastasis(χ2=5.021,P=0.025;χ2=7.289,P=0.007).The positive expression rates of Ki-67 and COX-2 in carcinoma tissue in the patients at Ⅰ+Ⅱ stage were significantly lower than those in the patients at Ⅲ+Ⅳ stage(χ2=8.570,P=0.003;χ2=11.409,P=0.001).The expressions of COX-2 and Ki-67 in colorectal carcinoma tissue of the patients had a positive correlation(r=0.520,P<0.05).The post-operation medium survival time of the patients were shortened with the increasing of the expression levels of Ki-67 and COX-2, and the 1-,3-,and 5-year survival rates of the patients with positive expressions of COX-2 and Ki-67 were decreased gradually.Conclusion: The abnormal expressions of COX-2 and Ki-67 play an important role in the occurrence and development of colorectal carcinoma and may be responsible for the occurrence,development,invasion and metastasis of tumor.The expressions of COX-2 and Ki-67 may be the reference indicators to judge the biological behavious of colorectal carcinoma.
