Detection of bcr/abl fused gene patterns in patients withchronic myelocytic leukemia by RT-PCR

摘要: 目的:提高RT-PCR检测慢性粒细胞白血病(CML)残留白血病细胞的敏感性,探讨最适逆转录酶用量、最适退火及延伸温度以及PCR周期数,确定bcr/abl的融合类型,并研究其与预后的关系。方法:通过改变PCR条件,将常规用量的逆转录酶减少到常规的1/4(5 U),退火温度由原来的50℃增加至60℃,反应周期数由原来的30周期增加到45周期,检测临床164例CML患者标本。结果:可使残留白血病细胞的检出率由原来的10-4提高至10-5~10-6。155例Ph染色体阳性的患者中发现bcr/abl融合基因类型,其中b3/a2型78例,b2/a2型54例,b3/a2和b2/a2两型均有者为23例;9例Ph染色体阴性病例中,b3/a2型1例,b2/a2型3例,两型均有1例,阴性4例(即Ph-bcr-CML)。 结论: 几乎所有CML患者都有bcr/abl融合基因,并存在不同类型,有少部分患者体内同时存在两类不同嵌合体的白血病细胞,即两种不同的突变细胞株,预示着患者预后差。

Abstract: Objective To increase the sensitivity of residual leukemic cells detection in chronic myelocytic leukemia (CML) patients with RT-PCR, the optimal dose of reverse transcriptase,optimal annealing,extension temperature,and PCR cycles were studied to confirm bcr/abl fused gene types. Methods Through changing the PCR conditions, the dose of reverse transcriptase was decreased to one-quarter of routine doses (5 U),the annealing temperature was increased from 50℃ to 60℃, and the number of reaction cycles was increased from 30 to 45. All 164 CML patients were examined. Results bcr/abl fused gene types were found in 155 patients with positive Ph chromosome,of all 78 cases were b3/a2 type, 54 cases were b2/a2 type, and 23 cases were both b3/a2 and b2/a2 types. Of 9 negative Ph chromosome, 1 case was b3/a2 type, 3 cases was b2/a2 type, 1 case was both b3/a2 and b2/a2 types,and 4 cases were negative (Ph-bcr-CML). Conclusion Almost all the CML patients can be found that they have bcr/abl fused gene, few have two kinds of chimera leukemic cells (two kinds of mutational cell lines).
