热量限制和运动对肥胖女性脂蛋白颗粒直径和分布的影响

Effects of caloric restriction and exercise on lipoprotein particle diameter and distribution of female obesity

摘要:

目的:探讨热量限制和运动对肥胖女性脂蛋白颗粒直径和分布影响,为肥胖患者制定特异性的治疗方案提供依据。方法:80名30~60岁肥胖女性(BMI>28 kg?m-2)随机分为对照组(CG,n=20)、热量限制组(DG,n=20)、运动组(EG,n=20)和热量限制+运动组(DEG,n=20)。DG和EG分别进行节食和运动干预,DEG则同时进行节食和运动,CG保持日常生活习惯不变,实验共8周时间。实验前后利用聚丙烯酰胺凝胶电泳测定脂蛋白颗粒直径。将待测血浆电泳后固定、染色、摄片,以标准蛋白直径的自然对数为纵坐标,以迁移距离为横坐标做标准曲线,根据待测标本在凝胶板上的迁移距离读出低密度脂蛋白(LDL)和高密度脂蛋白(HDL)颗粒直径。结果:与实验前比较,8周实验后DG LDL颗粒直径增加(P<0.05),小颗粒LDL(sLDL)比例减少(P<0.05),大颗粒HDL(lLDL)比例增加(P<0.05),HDL颗粒直径和分布差异无统计学意义(P>0.05);EG HDL颗粒直径增加(P<0.01),sHDL比例减少(P<0.05),lHDL比例增加(P<0.05),LDL颗粒直径和分布差异无统计学意义(P>0.05);DEG LDL和HDL颗粒直径均增加(P<0.01,P<0.05),sLDL和sHDL比例减少(均P<0.01),lLDL和lHDL比例增加(均P<0.01);CG 各指标差异均无统计学意义(P>0.05)。结论:单纯热量限制或运动只能部分影响LDL和HDL颗粒直径和分布,只有联合热量限制与运动才能同时改善LDL和HDL颗粒直径和分布,从而降低肥胖女性患者心血管危险水平。

Abstract:

Abstract:Objective  To explore the effects of caloric restriction and exercise on lipoprotein particle diameter and distribution of female obesity and provide evidence for obese patients to formulate special therapeutic schedule.
Methods Eighty female obese aged 30-60 years were randomly divided into control group (CG,n=20),diet group (DG,n=20),exercise group (EG,n=20) and diet+exercise group (DEG,n=20).The subjects in DG and EG performed diet and exercise interven
tion respectively and those in DEG simultaneously conducted two intervention style above,while those in CG maintained their routine lifestyle.The test duration was 8 weeks.The lipoprotein particle diameters were measured by polyacrylamide gel
electrophoresis before and after test. Fixation,staining and photograph were made after plasma electrophoresis.Standard curve was determined by taking natural logarithm of standard protein diameter as ordinate and migration lenth as abscissa.
The diameters of low density lipoprotein (LDL) and high density lipoprotein (HDL) particle  were figured out according migration lenth on gel board of the samples.Results Compared with before test,8 weeks after test,the LDL particle diameter was increased (P<0.05),the proportion of small low density lipoprotein (sLDL) particle was decreased (P<0.05) and the proportion of large low density lipoprotein (lLDL) particle was raised (P<0.05),the diameter and distribution of HDL had no significant change (P>0.05) in DG; the HDL particle diameter was elevated (P<0.01),the proportion of small high density lipoprotein (sHDL) particle was reduced (P<0.05) and the proportion of large high density lipoprotein (lHDL) particle was raised (P<0.05),the diameter and distribution of LDL had no significant change (P>0.05) in EG; both of the LDL and HDL particle diameters were  elevated (P<0.01 or P<0.05),the proportions of sLDL and sHDL particles were reduced (P<0.01) and the proportions of lLDL and lHDL particles were  raised (P<0.01) in DEG; all indices in CG showed no significant differences (P>0.05).Conclusion Caloric restriction or exercise could partly have impact on LDL and HDL particle diameters and distribution of female obesity while combination of caloric restriction and exercise may simultaneously improve lipoprotein particle diameter and distribution,thus reduce cardiovascular risk level of female obesity.
